  • Monday, March 26 - Choice of full gourmet salad box, full gourmet sandwich box, or gourmet half-sandwich/salad combination box. Delicious and filling! Choice of beverage included.

Instructions:

1) Download the Conference Registration Form (PDF file) to your computer and print it out

NOTE: To view the form, you will need to use Adobe Reader. If you do not already have Adobe Reader on your computer, you may download it for free from the Adobe website.

2) Complete the form, making sure to mark all the options you wish to purchase (as instructed).

3) Please include your payment with your completed registration form. Payments must be in US dollars. Acceptable forms of payment include checks, (international) money orders, and traveler's cheques, made payable to "University of Hawaii". Cash is also acceptable but generally recommended only if registering in person. Sorry, we cannot accept credit cards. No refunds will be given after February 15, 2007 (Written or email notification of cancellation received by February 15, 2007 will result in full refund minus administrative fee).

4) Send your completed form and payment in as instructed on the form. Do so by February 15, 2007 to enjoy special conference pre-registration rates!
